Please save time at your first appointment by downloading the appropriate set of forms, fill them out, and bring them with you.
Your therapist at Lighthouse Christian Counseling is looking forward to seeing you. To get the most out of your first session,
please fill out forms for your first appointment and bring them with you. You may also arrive 15-20 minutes early and fill them out in our reception area.
Please be assured we follow Texas state laws in regard to the confidentiality of your information you share with us.
1. Open the forms for your counselor. Each adult client will need an Adult Consent form and an Adult Intake form. For minors the necessary paper work is an Adult Consent form, a Minor Consent form and a Minor Intake form.
2. Print them on your printer.
3. Fill them out and bring them to your first appointment.

5. For Out of Network benefits with Jana Enlow, please follow the instructions for calling your insurance company before the first visit and download the “Questions for Insurance” form.
